Created, written and illustrated by Lemire, Sweet Tooth was first published by DC Comics under its Vertigo imprint in September of 2009. The comic follows Gus, who embarks on a cross-country journey with a hot-headed ex-hockey player Tommy Jepperd. Lemire's Sweet Tooth originally ran for 40 issues, concluding in January of 2013. Lemire later returned to write and draw a six-issue sequel series titled Sweet Tooth: The Return, which ran from November of 2020 to April of this year. With Vertigo now defunct, the title was published under the DC Black Label.

Netflix's adaptation is directed by Jim Mickle, who serves as co-showrunner alongside Beth Schwartz. Mickle and Schwartz both serve as writers and executive producers on Sweet Tooth, with other executive producers including Susan Downey, Robert Downey Jr., Amanda Burrell and Linda Moran.
