Star Wars: Shadow of Vader's Castle revealed a moment with little exploration in the Star Wars Galaxy and provided some information on what happened to Anakin on Mustafar before Padme and Obi-Wan arrived.

While it's only for a brief period of time, there was just enough for everyone to know what Anakin was feeling, what he was willing to do, and reveal his new greatest fear, demonstrating just how far he had fallen by that point.

While observing the fiery landscape of Mustafar, Anakin was spied on by a young Mustafarian boy named Kakan. There was no malicious intent behind the snooping, Kakan was young and impressionable and merely wanted to see a Jedi with his own eyes. At first, the sight of Anakin in his billowing robes lived up to Kakan's expectations. Then, Anakin turned to face the boy, and Kakan could see the hate and darkness in his eyes. Terrified, and recognizing that what he had seen was not a Jedi, Kakan fled with Anakin in pursuit. Not wanting any witnesses to this, Anakin was fully prepared to kill yet another child whose only crime was wanting to see the idols he worshipped. He would have done it too had it not been for the intervention of Lellis, an older Mustafarian.

Knowing they could not possibly fight someone with the Force, Lellis fiddled with the controls of the station, filling the room with highly hallucinogenic gas from the volcanoes. While the Mustafarians tried to escape, Anakin's mind was quickly affected by the toxins. He began seeing visions of Qui-Gon Jinn, the very first person Anakin cared about to die over the course of his life. Qui-Gon began criticizing Anakin, calling him weak. Anakin fought through this vision at first, believing himself to be more powerful than ever, but the visions did not stop there. As he boasted of his power, a demonic face of Yoda appeared, correcting that Anakin was only strong with the Dark Side and not worthy to be a Jedi anymore. The terrifying and accusing figure then stated the Jedi would destroy Anakin for what he had become.

Anakin was then surrounded by visions of Qui-Gon, Yoda, Ahsoka, Padme, and Mace Windu, and they all taunted him for his weakness. This pushed Anakin over the edge and he unleashed a force push strong enough to dispel the gas, but unintentionally help his targets escape him. They had also been suffering through the gaseous effects, seeing their greatest fears and falling off the platform. Anakin's force push had ironically saved their lives.

However, the visions Anakin saw were significant in their own way. It wasn't just a means of distraction, but an examination of where Anakin's priorities were at that moment. During this time, Anakin is at his most powerful, his future in the armor of Darth Vader is merely a shadow of what he could have become. The visions show that he still thinks he is not strong enough and perhaps more deeply, afraid that he wasn't strong enough to hold onto the Light Side.

What is even more chilling is the fact that weakness is his fear now. Mere days ago his greatest fear was losing Padme. While this had caused him to fall and serve Palpatine, it still came from a place of love. Now, this care and concern have been replaced by a selfish and petty need to become more powerful. So with this as the overriding fear in life, what he did to Padme shortly after comes as no true surprise.
