In the Sailor Moon universe, Usagi Tsukino has had quite a few transformations throughout the story. Often, her transformation item is destroyed and she needs a replacement or she needs an upgrade to take on a new type of enemy.

With each transformation, she seems to obtain new powers, sometimes new powerful attacks, healing abilities, and even powers over nature itself. Some of these transformations also give her deeper connections to certain allies, especially the characters who gave her each new transformation. And, depending on whether you are watching the anime or reading the manga, she could seem to have a whole slew of different abilities.

5 Moon Crisis

Sailor Moon Chibusa

At this point in the series, Usagi and Chibiusa's powers seem to be symbiotic. Usagi seems to need Chibiusa in order to summon her attack. Likewise, Sailor Chibi Moon seemed to mostly exist as a source of support for her future mother. The anime even highlights this by having them share their transformation sequence.

It's a little unclear if Usagi still has her healing powers. She was able to use "Moon Gorgeous Meditation" to transform the vampire Lyrica into a normal human, but she had to upgrade to Eternal Sailor Moon to heal Queen Nehellenia.

While Usagi's new transformations can generally be seen as upgrades, this one can be argued to have been a transformation that simply gave them the attacks directly needed to fight the Lemures.

International versions aside, this is also one of the few transformations that didn't use the word, "Power."

4 Moon Prism Power

This was the transformation that started it all, allowing Usagi to first become Sailor Moon herself. In her first transformation, Usagi seemed to have powers coming out of the woodwork. Even her breaking down and crying could turn out to be a powerful attack that saves the day.

In fact, Usagi gets a lot of weapons during this time, from a transformation pen to a mask that helps her see people in danger that she seems to discard. Whether this is due to the limited use of these powers or simply Usagi wasting her potential is up for debate.

This version is explicitly weaker than Usagi's full potential as she has to transform into Princess Serenity to finally take on Queen Beryl after she fused with Metallia. And, even then, she still needed her friends' help to vanquish her enemy.

While Usagi did obtain many useful attacks and powers during this phase, she didn't necessarily lose them after upgrading, as she could still be seen using them in later arcs, from attacking with her tiara to even using her disguise powers. This is also one of the few transformations that explicitly comes with a healing spell, although Usagi seems to retain this power in some form or another.

3 Moon Cosmic Power

Usagi obtains this power during the third story arc as she has to contend with the Witches 5. In the anime, she explicitly starts to lose her previous powers after a Daimon, a monster-of-the-day, attacks her brooch.

With the help of the Holy Grail, Usagi is also able to transform into Super Sailor Moon, which gives her a tremendous increase of her powers. After she sacrifices the Grail in a bid to save Hotaru, she is able once again to unlock this form with the help of the other Scouts sharing their powers with her. She was able to use this form to save Hotaru.

However, it should be noted that this is one of the few times, in the anime, at least, where Sailor Moon wasn't the one to take on the main villain of the arc, Pharaoh 90. Instead, Sailor Saturn appeared to be the one to save the day, not to mention destroying Mistress Nine from within. Even in the manga, Sailor Saturn was the one to defeat Pharaoh 90, although there he seemed to have been sealed away instead of destroyed. Then again, it could be less Sailor Moon, or Super Sailor Moon, being unable to take on the villain and more it just being Sailor Saturn's time to shine.

In the manga, she also had to transform into Neo-Queen Serenity to heal the damage done to the city, suggesting she couldn't have done the job as a mere Sailor Scout.

2 Moon Crystal Power

In the anime, Usagi gets this new transformation from her mother, Queen Serenity, after her previous powers seem to fail against the Cardians, with one even managing to destroy her brooch. She notably obtains "Moon Princess Halation."

Usagi was once again shown to have retained her healing powers in this form, being able to transform the Four Spectre Sisters into normal humans, in the anime version of the story. She didn't even need a special spell or attack to accomplish this, just wish for it using the Silver Crystal.

Admittedly, in the final battle against the Death Phantom, Sailor Moon did have to transform into Neo-Queen Serenity, but this could be seen as her future self giving a helping hand. And while the other Sailor Scouts shared their power with her, this could have just been a precaution.

This transformation also gave Usagi an advantage against natural disasters, since she was able to break down a rockslide and save Chibiusa's dinosaur friends using the Cutie Moon Rod.

As mentioned, a Daimon was able to destroy the brooch, but it's a little unclear if this was exclusive towards this transformation or something that could have happened to any brooch, as it also happened to her Prism Power brooch.

1 Moon Eternal Power

This is one of the few times Usagi once again had healing powers, being able to change Phages back into normal human beings, using her attack, "Starlight Honeymoon Therapy Kiss." This could also be used to vanquish more sinister villains, such as Queen Nehelennia in the manga. She also obtains her most powerful attack in the anime: "Silver Moon Crystal Power Kiss."

There were some limits to this transformation, however. Usagi obtains wings in this form, but they don't seem to give her the ability to fly and they can actually be a nuisance, such as giving the villains something to pin her down with. She also notably loses her tiara. In one story when it would have certainly come in handy, she had to improvise with a nearby pizza. It's also notable that she has to transform into Princess Serenity, as well as an angel, to take on Galaxia during their final showdown.

It should be noted that some of her powers were shown to be exceedingly stronger in the manga than the anime. In the former, Usagi was even able to take on the other Sailor Scouts, after having been enslaved by Galaxia, in a single attack. She was also able to revive them when they should have been obliterated. Even in the anime, it was implied she could have saved the Sailor Animamates if things had played out differently.
