My Hero Academia may be about the journey of young U.A. students to become heroes, but it doesn't take long for the anime to make it clear that the journey will cost them. Protagonist Izuku Midoriya suffers from many injuries while learning how to master his Quirk. His hands will forever carry the scars of his choices.

Izuku isn't the only one in this situation. Pro Heroes and villains alike have had to suffer for their ideals. While so far, there haven't been a lot of deaths in the anime, there have been plenty of injuries, some of them major.

10 Pro Hero: All Might's Fight With All For One Costs Him His Organs And His Muscle Form

My Hero Academia All Might United States of Smash

The Symbol of Peace, All Might, is the hero everyone looks up to and the pillar sustaining hero society. But Toshinori Yagi is just a man, and even the strongest pillars can crumble under too much pressure. A fight with his nemesis, All for One, costs All Might half his respiratory organs, as well as his stomach.

His true form deteriorates, and he becomes almost skeletal. He is able to hold onto his muscle form for a few hours daily, but the amount of time deteriorates after the battle with the USJ nomu. After fighting All for One for a second time, he is forced to retire, the spark of One for All extinguishing inside him.

9 Villain: All For One Is Mortally Injured By All Might And Is Forced To Rely On Life Support

All Might's injuries may have been serious, but his nemesis isn't unscathed either. The sinister All for One loses his face and becomes blind. He is able to see through an infrared Quirk, but he has no eyes. He has trouble breathing and wears a mask as life support. His Quirks are the only thing that save his life, but even then, All for One knows he's living on borrowed time. To this end, he commands Doctor Kyudai Garaki to build him a new vessel--which he ultimately finds in Tomura.

8 Pro Heroes: Eraserhead And Thirteen Are Badly Hurt During The Villains' Attack On The USJ

Aizawa and Thirteen fighting at USJ MHA

The young students of 1-A are very excited when they first arrive at the Unforeseen Simulation Joint. Unfortunately, their field trip turns into a disaster when a group of villains led by Tomura Shigaraki infiltrates the USJ. Eraserhead is able to hold his own against the lower grade villains, but his elbow is disintegrated by Shigaraki. He is then attacked by the nomu, and the damage he takes from the creature permanently affects his Quirk. Pro Hero Thirteen tries to protect the students from Kurogiri, but Kurogiri turns her Quirk against her, and Thirteen's back is torn apart by the power of Black Hole.

7 Villains: Tomura Shigaraki And Kurogiri Are The Targets Of Kyudai Garaki's Vicious Experiments

My Hero Academia _ Shigaraki, Kurogiri, and the Nomu at USJ

As much damage as Tomura and Kurogiri do throughout their villainous career, they don't exactly live a pain-free existence. Kurogiri is a nomu, his body created primarily out of the corpse of Oboro Shirakumo, with some other corpses and Quirks thrown in.

Tomura is just as much of a victim as he is a villain. Manipulated by All for One from an early age, he becomes nothing more than a tool in his mentor's schemes. He goes through incredibly painful procedures at Dr. Garaki's hands. And that doesn't even include what he and the League of Villains have to go through while fighting Gigantomachia and the Meta Liberation Army.

6 Pro Heroes: Ingenium And Native Are Among The Many Heroes Crippled By The Infamous Hero Killer

Tenya Iida sees Tensei in hospital in My Hero Academia

Chizome Akaguro is a very interesting villain in that he makes some good points. Hero society is indeed corrupt and has many flaws. But the method he chooses to fix that doesn't really help.

Many good heroes are crippled and killed in the Hero Killer's mad quest to purge society from the 'fakes'. Native is also ambushed in Hosu and it is only Tenya's intervention that saves his life. It doesn't go that well for the young hero either, and without his friends Izuku and Shoto, he'd have undoubtedly been killed.

5 Villain: The Hero Killer Stain's Punctured Lung Interrupts Him Mid-Speech

Stain unconscious MHA

Even when he's not wielding a sword and physically attacking heroes, Stain is terrifying. His final speech shocks even Endeavor, his murderous aura too potent for anyone to challenge. But the battle with the three students of 1-A does take its toll on him. His broken ribs puncture his lung, knocking him out mid-speech. Compared to all the damage he's inflicted, it's not that serious, but it's an almost anticlimactic end for the intense battle.

4 Pro Hero: Best Jeanist Loses A Lung To All For One's Air Cannon

Best Jeanist clashes with All For One and is defeated

During the Kamino Incident, when All for One appears, Best Jeanist is able to throw his fellow heroes out of the way using his Quirk Fiber Master. Unfortunately, no one arrives in time to save him and All for One shoots him with Air Cannon. After his recovery, his conversation with Hawks reveals that the injury was so serious one of his lungs had to be removed. The anime also suggests that Hawks kills the hero to earn the League of Villains' trust, but that turns out to be a lie.

3 Villains: Overhaul And Mr. Compress Lose Their Hands To Each Other

overhaul

Overhaul is perhaps one of the most hated villains in the anime. In his fanaticism and viciousness, he uses young Eri to create his Quirk-canceling bullets. But Eri isn't his only target. When the League of Villains' meeting with Overhaul goes south, Overhaul kills Magne and obliterates one of Mr. Compress's hands.

The League isn't happy at all. After Overhaul is defeated, Shigaraki hunts him down. He decays one of Overhaul's hands, while Mr. Compress claims the other.

2 Pro Hero: Endeavor Is Left Scarred By His Battle With The High-End

Endeavor's victory over High End MHA

After All Might's retirement, Endeavor becomes the Number One hero. But the villains are still out there and ready to strike. A High-End nomu targets Endeavor and Hawks, and while Hawks works to save the civilians and provide backup, it's up to Endeavor to defeat the creature. The two heroes are victorious, but Endeavor is grievously wounded. For the most part, he manages to recover, but he is left with a massive scar on his face that will never heal.

1 Villain: Dabi's Scars Are The Result Of His Abusive Upbringing At Endeavor's Hands

Dabi releases Blue Flames in My Hero Academia

Endeavor's injury is perhaps karmic, as his abuse of his children and his wife led both Touya and Shoto to be permanently scarred. Touya's injuries are particularly gruesome. With his father so fixated on defeating All Might, Touya tries to overcome his body's incompatibility with his Quirk. It doesn't work, and his desperate determination causes a Quirk accident that leads to the scars he bears to this day.
