Concept artist Jerad Marantz posted vivid concept art for Doctor Strange (2016) featuring an alternate design for the Mindless Ones.

The hulking cyclopean monstrosity is lined with shimmering translucent cracks that imply an at least partially hollow center. A single brilliant red searchlight of an eye blazes from a gap in its receding head and powerful arms hang poised to grasp and smash from the creature's size, like the coiled muscle of a snake before it strikes.

The Mindless Ones were a frequent threat in the Marvel canon, especially the Doctor Strange comics. Brought to the Dark Dimension by an ill-fated wizard, they have primarily served as the monstrous pets of Dormamu, although not even he can fully control them. They've faced off not only against Doctor Strange himself but gone toe to toe with some of the biggest names in the Marvel Universe, including Spider-Man, The Fantastic Four and even Deadpool.

Marantz is no stranger to the Marvel Cinematic Universe. In addition to Doctor Strange, he's also worked on Black Panther and Guardians of the Galaxy. He specializes in creature design, providing concept art and character layout for various monsters throughout cinema. Examples include a foreboding take on Taserface of GotG Vol. 2 fame, as well as the threatening critters of the short-lived Swamp Thing series. His work appears in video games, as well, such as the enduring Gears of War franchise.
