Created by Stan Lee and Steve Ditko, Clea made her first Marvel Comics appearance in 1964 with Strange Tales #126. Screenwriter Michael Waldron recently discussed the decision to introduce Clea, who is Strange's wife in Marvel Comics, at the end of Multiverse of Madness. According to Waldron, her debut was always part of the plan. "We always knew we wanted to introduce Clea, who in the comics is you could say the great love of Doctor Strange, but really in a lot of ways is his formidable equal as a sorcerer herself," Waldron said.

"Her backstory is fascinating," he continued. "She's the niece of Dormammu, the giant floating head from the first movie. And so, they have a lot of great adventures in the comics, and we knew we wanted to introduce her, but it felt like we had to close the book to some extent on his love story with Christine Palmer, Rachel McAdams' character."

Doctor Strange in the Multiverse of Madness picks up after the events of Spider-Man: No Way Home and ties directly into two of Marvel's Disney+ series, WandaVision and Loki, both of which dealt heavily with the multiverse. However, while the Doctor Strange sequel will delve further into the multiverse, writer Michael Waldron explained that the multiverse will not be used as a plot device or a "MacGuffin," and instead will act as the film's emotional core.

Following the success of Spider-Man: No Way Home, it was reported that Multiverse of Madness would have "more fun with the multiverse," with reshoots including even more cameos and character introductions than originally intended. Among the possible cameos are Patrick Stewart as Professor X, an appearance from Captain Carter, who was introduced in Marvel's What If...? series, and the live-action debut of America Chavez, played by Xochitl Gomez.
